Thanks everybody for your input. For anybody reading
this in the future, the problem was fixed by placing
the full url path in the form specified above:
jdbc:mysql://<host>:<port>/<database name>. Apparently
this is needed even if you are running the app from
the same machine as the DB.

Well, jeepers dude, I can maybe agree that it might make sense for it to default the host part to localhost, but what did you expect it to do for the port? What "port" is your app running within (it isn't). Or what "default port" could it possibly use?

You ought to award those Dukes, fulfilling your implicit promise. I'd vote for Monika.

The MySQL default port is 3306, according to the docs.

Hmmm, well maybe the driver could have been made smarter then, if it's set in stone to have always been and always will be 3306 no matter the MySQL version. Ok, so never mind my jeepers dude remark.

That's the default value that MySQL will use if the admin takes no further action, but I could see where an admin might want to choose another port if there was a conflict OR to confound any hackers who might take advantage of their knowledge of the default port. Better to leave it optional in the driver, IMO.

I don't know about books, but here's a tutorial. It's about JDBC in general, but everything in there should apply to MySQL.
